In the election campaign you hear a lot about health insurance, and the 47 million Americans who do not have it.
But for people in rural America, just having access to doctors and clinics can be just as much of a problem.
This week, Inside USA travels to the Appalachian mountains, where some counties have seen a drop in life expectancy - a shocking statistic in a country as rich as the US.
There we look at how many are fighting back against the system, and discover a network of free clinics and health services for the poor, run by people who are filling the gaps in the system with the sheer force of their own commitment.
